Instructions for Completing the Admission Application for the Physical Therapy Graduate Program

Youngstown State University students may be provisionally accepted into the MPT Program as undergraduates during the Summer semester of the senior year. They are not, however, admitted as graduate students until they have received their bachelor’s degree, completed their application for graduate program admission, and been accepted and admitted formally to the School of Graduate Studies. Students who have already received the bachelor’s degree prior to application are admitted as graduate students. Students who complete the bachelor’s degree at the end of the Summer semester will be admitted as graduate students effective at the start of the Fall semester.

You must, however, complete and submit a graduate school application form prior to the deadline set by the department of Physical Therapy.

The Level I, DPT Foundation Courses (which are graduate level) are normally taken in undergraduate status. They are counted as part of the DPT Program, but not as part of the graduate transcript. They are considered waiveable prerequisites that count toward the accredited physical therapy program requirements. You will not be charged graduate tuition for these courses if you are registered as an undergraduate.

Please follow the following instructions when completing the Application for Graduate Program Admission:

1. If you have received a bachelor’s degree prior to the start of the Summer I term, when the program initiates: The term in which you wish to enter is marked – Summer.

2. If you will be granted a bachelor’s degree prior to the beginning of the Fall term (and your faculty advisor has verified that you will receive the degree): The term in which you wish to enter is marked – Fall.

3. If you will complete your bachelor’s degree at the end of the Fall semester of your senior undergraduate year (and your faculty advisor has verified that you will receive the degree): The term in which you wish to enter is marked – Spring.

All other sections of the application to the MPT graduate program are completed as required!

If you have never attended the Graduate School at YSU, please return this application with a $30 nonrefundable application fee to the Bursar's Office.
